Client Follow-Up And Close-Out Process

CLIENT FOLLOW-UP AND CLOSE-OUT PROCESS

The initial Client Training Agreement will contain a section requesting that the client report the completion of the intervention, as well as his or her post-training employment results.

There will be a twelve (12) week follow-up period during which time this information will be tracked by the MHRDA staff.

The information required, as per the MHRDA, for each client is as follows:

  • Return to school,
  • Registration in another program,
  • Employer,
  • Type of employment (employment with an employer or self-employment),
  • Occupation,
  • Whether employment is part-time or full-time,
  • Date of return to work, and
  • Earnings.

Completion and follow-up information must be documented and included in the file. The following are examples of the information that should be collected:

COMPLETION

  • Copies of certificates, diplomas, etc.,
  • Certificates of completion,
  • Final Institutional Transcripts,
  • Final Employer Report (in case of employment-based interventions), and/or
  • Final Project Report (in case of project-based interventions).

POST-INTERVENTION RESULTS

Employment

  • Copy of pay stub, employment contract, etc., and/or
  • Self-Employment Declaration Form.

Return To School/Registration In Another Program

  • New Application to return to funded studies,
  • Training Institution Acceptance Letter/copy of paid registration fees, and/or
  • Letter of Alternate Program Sponsorship.
