Client Follow-Up And Close-Out Process
CLIENT FOLLOW-UP AND CLOSE-OUT PROCESS
The initial Client Training Agreement will contain a section requesting that the client report the completion of the intervention, as well as his or her post-training employment results.
There will be a twelve (12) week follow-up period during which time this information will be tracked by the MHRDA staff.
The information required, as per the MHRDA, for each client is as follows:
- Return to school,
- Registration in another program,
- Employer,
- Type of employment (employment with an employer or self-employment),
- Occupation,
- Whether employment is part-time or full-time,
- Date of return to work, and
- Earnings.
Completion and follow-up information must be documented and included in the file. The following are examples of the information that should be collected:
COMPLETION
- Copies of certificates, diplomas, etc.,
- Certificates of completion,
- Final Institutional Transcripts,
- Final Employer Report (in case of employment-based interventions), and/or
- Final Project Report (in case of project-based interventions).
POST-INTERVENTION RESULTS
Employment
- Copy of pay stub, employment contract, etc., and/or
- Self-Employment Declaration Form.
Return To School/Registration In Another Program
- New Application to return to funded studies,
- Training Institution Acceptance Letter/copy of paid registration fees, and/or
- Letter of Alternate Program Sponsorship.
