Documentation Of Monitor Findings

DOCUMENTATION OF MONITOR FINDINGS

The MHRDA Provincial Manager must document his or her findings. Notes must be kept in each Region’s file held by the MHRDA Provincial Coordinator concerning every monitoring visit. This will establish the timeline of significant events and provide a record of the program’s performance and program issues and results. It will also help the MHRDA Provincial Manager keep track of any suggestions made for corrective action. These notes also serve to guide anyone who may subsequently fill the role of MHRDA Provincial Manager.

A written summary of the monitor findings must also be provided to the MHRDA Program Coordinator, and in the case of Service Delivery Organizations, to the RETC.
